You can now request a non-emergency appointment for your family memeber at Specialty Hospital’s online (for all emergencies, please call ....). To use this online tool, simply:

• Indicate below whether you are a new or returning patient.
• Complete the form. Note: Information marked in RED must be submitted in order to process your appointment request.
